Requirements

If you have employees you must provide proof of
workers compensation insurance naming Premier Communities as Certificate
Holder. If you have no employees you must sign a statement to that
effect.

You must provide proof of general liability insurance
in the amount of $1 million, naming Premier Communities as Certificate
Holder.

If the types of services you provide require a
contractor's license you must provide a copy of your license.

Completed W-9 form for tax reporting purposes as required by the
IRS.

Services provided are frequently inspected for
thoroughness

Homeowners are frequently surveyed for their
satisfaction with your services.

Services may be suspended or terminated if any
deficiencies or complaints are noted.
